- This report provides an in-depth description of Ontario health system use by four vulnerable groups: (1) older women; (2) community-dwelling older adults and dementia; (3) medically complex home care clients discharged from acute care hospitals; and (4) older adults newly placed in long-term care. The report allows for visual comparisons of each group's health and functional status, health service use over time, and access to caregiver support.
