This handy resource lists suicide warning signs and risk factors relevant to clinicians working in rural communities.
 

Suicide Assessment Five-Step Evaluation and Triage (SAFE-T) guide

This quick guide for clinicians summarizes the five steps of suicide risk assessment. It was adapted for rural clinicians based on material developed by the U.S. Department of Veterans Affairs. 
 

Safety Planning guide

This quick guide for clinicians describes step-by-step how to work with patients at risk for suicide to develop a safety plan. It includes questions to ask patients to elicit the necessary information. This guide was adapted for rural clinicians based on material developed by the U.S. Suicide Prevention Resource Center. 
 

Patient Safety Plan Template

The clinician and the patient complete this patient safety template together, which the patient then uses to reduce suicide risk. It was developed by the U.S. Suicide Prevention Resource Center.

 

Suicidality Treatment Tracking Log

This log can be used to track and monitor suicide risk in patients with identified risk of suicide-related behaviour. It was developed by the U.S. Suicide Prevention Resource Center.
